Cystic fibrosis (CF) is the paradigm case for multi-layer, multi-organ biological displacement. Caused by mutations in the gene encoding cystic fibrosis transmembrane conductance regulator (CFTR), CF simultaneously displaces the lung, pancreas, liver, gut, and reproductive system from their respective ground states through a single causal event: loss of CFTR function at every epithelial surface where it is expressed.

We present a comprehensive unit on CF through the displacement framework, tracing the disease from its genetic origins through organ-system pathophysiology, the microbiome as a biological wrong attractor, the mental health burden of accumulated displacement cost, and the treatment landscape including CFTR modulators as displacement-reduction interventions. We formalize total CF displacement cost as, and identify residual displacement remaining even after transformative modulator therapy as the target for next-generation combination approaches.

CF is not a lung disease with complications. It is a total-body epithelial displacement disorder in which the lungs carry the highest displacement cost and the framework unifies its otherwise bewildering multi-organ presentation under a single mechanistic account.
