Secondary Gain as Illness-Identity Wrong Attractor: The Sick-Role Architecture, Suffering-as-Resource Basin, and the Return to Functional Selfhood

Secondary gain — the psychological attachment to a sick-role identity that provides care, exemption from demands, and social meaning, making recovery feel threatening — is here formally reconceptualized as a wrong attractor W_SG in the landscape of functional selfhood. This paper approaches the pattern with care: the suffering is real, the secondary gains are real, and the process is largely unconscious.

The wrong attractor is not malingering (deliberate simulation) but genuine psychological attachment to illness as organizing principle. The healthy ground state S^0 is defined as functional selfhood: identity, worth, and relational connection organized around one's capacities, contributions, and genuine self — with illness present when it is, without being constitutive. Three interlocking mechanisms maintain the basin: the care-access function (illness provides access to nurturing that is not otherwise available — recovery threatens loss of the only reliable source of connection), the demand-exemption function (the sick role legitimizes exemption from demands that feel overwhelming — recovery would require confronting what illness protects against), and the identity-organization function (illness provides a coherent identity, community, narrative, and role — recovery threatens dissolution of who one is).

The recovery paradox is central: improvement can produce symptom return — the system resists success because success threatens secondary gains.
